Cancer Evaluation/FQHCs/Organizations
Cancer patient evaluation for social determinants of health
HCMRI has dedicated resources and specialized MD/PhD expertise to design critical modifications to electronic health records at clinics, academic health institutions, and hospitals to systematically assess patients for social determinants of health.
This critical transformation automatically activates continuation support from a patient navigator, who connects Hispanic patients with community resources and facilitate coordinated care through partnerships with primary care clinics & physicians, community organizations, and Federally Qualified Health Centers (FQHCs) that provide comprehensive health care services to the Hispanic community.
Conduct scholar-led research
Community driven cancer projects with HCHF scholars
Hispanic Cancer Medicine Research Institute (HCMRI) voluntarily lends expertise and guidance through assistance to the Hispanic Cancer Health Foundation (HCHF) which coordinates and provides mentoring and support for clinical oncology, surgical oncology, and cancer medicine research scholars.
HCMRI research scholars work collaboratively to evaluate potential educational and sociodemographic barriers to obtaining survivorship care.
